Grant Application Procedure

Please provide the following required information when submitting a Bemis Company Foundation Grant Request.

1. Name of organization and amount requested. Include a description of your organization, its mission and goals, programs and activities service statistics, etc. Also include the number of full-time, part-time, and volunteer staff.
2. Description of the program or project objectives for which the grant is sought.
3. Number of people served by this program or project (or by your organization when seeking general operating support). Include geographic area served.
4. Details as to how the objectives are to be attained. If you are a service organization, include your method for measuring the effectiveness of your activities, as well as your success rate with this program in the past.
5. Budget for proposed program or project - please include on a separate sheet.
6. List of secured and pending donors with specific amounts donated for this program. Also include a list of donors to your organization’s other programs or annual support within the past twelve months, including the specific amounts donated (only include corporate and foundation donations $1,000 and above) - please include on a separate sheet.
7. Percent of organization’s total grant money received that comes from the government, and the percent of organization's operating budget that is spent on fund-raising and administration.
8. Organization’s Current Operating Budget showing revenue sources and expenses - please include on a separate sheet.
9. Most recent year-to-date financial statements (balance sheet or statement of activities and functional expenses).
10. Copy of organization’s most recent audit.
11. Officers and Board Members with affiliations - please include on a separate sheet.
12. Statement that the organization has tax-exempt status under Section 501(c)(3) of the Internal Revenue Code and that a contribution to it is tax deductible.
